dc.description.abstract"En este trabajo fueron analizados dos de los principales factores determinantes de la eficacia de la vacunación profiláctica contra la infección por papilomavirus humanos tipo 16 (HPV16) y el desarrollo cáncer cervicouterino (CaCu) en el centro de México: el fondo genético de los alelos del complejo principal de histocompatibilidad de la población sujeta a vacunación y la variación de la proteína L1 de HPV16. Las secuencias nucleotídicas completas del marco de lectura abierto (ORF) del gen L1 (1509 pares de bases) fueron obtenidas de 26 aislados de HPV16 provenientes del cuello uterino de mujeres de los estados de San Luis Potosí y Guanajuato, México, con infección única por HPV16. Los ORF tuvieron una variabilidad nucleotídica del 3.9% y aminoacídica del 2.2%. El 81% de los aislados de HPV16 fueron del subtipo Europeo (E) y 15% del subtipo Asiático-Americano (AA). Los efectos estructurales de la variabilidad de la proteína L1, evaluados en la base de datos Swiss Model, fueron: 1) aumento del tamaño de la hélice 5 (26 casos), 2) pérdida de las hélices 210-213 (un caso) y 3 (dos casos), y 3) pérdida de la capacidad de autoensamble de los pentámeros (dos casos). El aporte del fondo genético de las mujeres fue evaluado mediante tipificación de los alelos de los loci HLA DQA1 y DQB1 con la técnica RSCA en 55 mujeres con infección por HPV16 (entre ellas estaban incluidas las 26 portadoras de los aislados de los ORF secuenciados). En ellas estuvieron sobrerrepresentado dos alelos DQA1: DQA1*0301, ampliamente reconocido como un alelo de susceptibilidad a CaCu (34% de las mujeres eran homocigotas), así como DQB1*0603, considerado como un alelo protector. El 24% de las pacientes tenían las combinaciones DQA1*0301/DQB1*0302 y QA1*0501/DQB1*030101, que se saben asociados a la susceptibilidad a CaCu. No hubo asociaciones significativas entre los alelos MHC II y los subtipos E y AA."
dc.description.abstract"In this work two major factors determining the efficacy of prophylactic vaccines against human papillomavirus type 16 (HPV16) infection and cervical cancer (CC) were assessed for Central Mexico: genetic background of the major histocomaptibility complex in the population subject to vaccination and variation of the L1 protein in circulating HPV16 viruses. Nucleotide sequences of the open reading frame (ORF) from the L1 gene (1509 base pairs) were obtained from 26 HPV16 isolates derived from the cervix of women residing in the states of San Luis Potosí and Guanajuato, Mexico, infected only by HPV16. The ORFs had 3.9% nucleotide and 2.2% amino acid sequence variability. Eighty-one percent of the HPV16 isolates were of the European (E) subtype and 15% of the Asian- American (AA) subtype. The structural effects of L1 protein variability, assesed by the Swiss Model software and database, were: 1) increased length of the helix 5 (26 cases), 2) loss of helices 210-213 (one case) and 3 (two cases), and 3) loss of L1 pentamer selfassembly (two cases). The contribution of the genetic background of the patients was assesed by typing the HLA DQA1 and DQB1 loci with the RSCA technique in 55 women infected by HPV16 (including the 26 ones of the sequenced ORFs). Two DQA1 alleles were over-represented in them: DQA*0301, widely recognized as a CC susceptibility allele (34% of the women were homozygous), and DQB1*0603 considered as a protective allele. Twenty-three percent of the patients had the two predominant haplotypes (DQA1*0301/DQB1*0302 and DQA1*0501/DQB1*0301), known to be associated with CC susceptibility. No significant association was found between MHC II alleles and the E and AA subtypes. Antigenic variability of the circulating viruses was correlated with the patients’ genotype by predicting the L1 protein epitopes capable of binding the MHC II glycoproteins produced by the alleles DQA1*0301/DQB1*0302, DQA1*0501/ DQB1*0201 and the allele DQB1*0301 with the RankPep software program. The immunologic effects of L1 variability fell in two categories: 1) epitopes exposed on the surface of the viral capsid and 2) antigenic potential of each viral isolate depending on the affinity of its individual epitopes. The effects of variability on the first category were: loss of epitopes (two epitopes from two isolates), changes in the D-E connecting loop (eight changes in five isolates) and changes in the H-I connecting loop (two epitopes from two isolates)."
